Summary
Disopyramide can cause hypoglycemia on its own, and this can be severe in some patients, especially the elderly and those with renal impairment. Combined with a sulfonylurea such as glyburide, the risk of low blood glucose is increased.
What this interaction means in plain words
Disopyramide and Glyburide-metformin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Disopyramide belongs to Cardiovascular; Glyburide-metformin belongs to Endocrine.
Why it happens
Disopyramide enhances pancreatic insulin secretion, which adds to the sulfonylurea effect; the combination is pharmacodynamically additive.
What to do
Monitor blood glucose closely when starting disopyramide and teach the patient to recognise hypoglycemia symptoms. Reduce the glyburide dose if hypoglycemia occurs, or consider an alternative antiarrhythmic.
What to ask your pharmacist
