Summary
Both drugs depress cardiac conduction and contractility, so combined use can cause excessive negative inotropy, bradycardia, hypotension, and heart failure. Disopyramide may also prolong the QRS and QT intervals, adding to the risk of arrhythmia.
What this interaction means in plain words
Disopyramide and Propranolol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Disopyramide belongs to Cardiovascular; Propranolol belongs to Cardiovascular.
Why it happens
Additive negative inotropic and conduction effects from sodium channel blockade and beta-adrenergic blockade.
What to do
Avoid the combination unless clearly necessary, and if used, monitor ECG (PR, QRS, QT), heart rate, and blood pressure closely. Use reduced doses and watch for signs of heart failure.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
