Summary

Dofetilide has a high risk of torsades de pointes, which is increased by hypokalemia and hypomagnesemia. Indapamide can cause these electrolyte disturbances, and thiazide-type diuretics may also reduce dofetilide clearance.

What this interaction means in plain words

Dofetilide and Indapamide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Dofetilide belongs to Cardiovascular; Indapamide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive QT prolongation with diuretic-induced electrolyte loss.

What to do

Avoid unless electrolytes are closely controlled, and monitor ECG, renal function, potassium and magnesium. Consider a potassium-sparing alternative or a different antihypertensive.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
