Summary
Dofetilide is a potent QT-prolonging antiarrhythmic with a substantial torsades risk. Triptorelin-induced androgen deprivation may further lengthen the QT interval, adding to the danger.
What this interaction means in plain words
Dofetilide and Triptorelin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dofetilide belongs to Cardiovascular; Triptorelin belongs to Endocrine.
Why it happens
Additive pharmacodynamic QT prolongation; dofetilide blocks IKr and androgen suppression may reduce repolarization reserve.
What to do
Avoid if possible; if unavoidable, continue dofetilide only with inpatient initiation protocols, continuous ECG monitoring and normal electrolytes. Reassess QTc after each dose change and consider an alternative to dofetilide.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
