Summary

Serotonin syndrome has been reported rarely when 5-HT3 antagonists are used with SSRIs such as fluoxetine. Fluoxetine also inhibits CYP2D6, which is involved in metabolism of hydrodolasetron, and it can modestly prolong QT.

What this interaction means in plain words

Dolasetron and Fluoxetine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Dolasetron belongs to Gastrointestinal Tract; Fluoxetine belongs to Central Nervous System.

Evidence level: Low

Why it happens

Possible additive serotonergic effects plus CYP2D6 inhibition that may raise hydrodolasetron exposure and QT effect.

What to do

Monitor for signs of serotonin toxicity and consider an ECG if other QT risk factors are present. Use a different antiemetic if the patient is at high arrhythmia risk.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
