Summary
Both drugs prolong cardiac conduction and the QT interval. Giving them together adds to the risk of torsades de pointes and other serious ventricular arrhythmias, especially with electrolyte disturbances or structural heart disease.
What this interaction means in plain words
Dolasetron and Propafenone can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Dolasetron belongs to Gastrointestinal Tract; Propafenone belongs to Cardiovascular.
Why it happens
Additive pharmacodynamic effect on cardiac repolarization and conduction (the active metabolite of dolasetron, hydrodolasetron, blocks sodium and potassium channels).
What to do
Prefer an antiemetic with less cardiac effect, such as a different class, if propafenone is needed. If unavoidable, correct potassium and magnesium first and monitor the ECG.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
