Summary

Lopinavir is co-formulated with ritonavir, a strong CYP3A4 inhibitor, which raises dronedarone levels. Lopinavir/ritonavir also prolongs QT and PR intervals, adding to arrhythmia risk.

What this interaction means in plain words

Dronedarone and Lopinavir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Dronedarone belongs to Cardiovascular; Lopinavir belongs to Anti Virals.

Evidence level: Moderate

Why it happens

CYP3A4 inhibition reduces dronedarone clearance, with additive cardiac conduction effects.

What to do

Do not combine; use an alternative antiretroviral regimen or antiarrhythmic.
