Summary
Lopinavir boosted with ritonavir strongly inhibits CYP3A4, which is the main pathway clearing cisapride. Raised cisapride levels can cause serious QT prolongation and torsades de pointes.
What this interaction means in plain words
Cisapride and Lopinavir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Cisapride belongs to Gastrointestinal Tract; Lopinavir belongs to Anti Virals.
Why it happens
Inhibition of CYP3A4-mediated cisapride metabolism by the ritonavir component, with lopinavir's own QT effect.
What to do
Do not coadminister; use an alternative prokinetic in patients on lopinavir/ritonavir.
What to ask your pharmacist
