Summary

Lopinavir is boosted with ritonavir, a strong CYP3A4 inhibitor, which markedly increases eletriptan exposure. This raises the risk of serious vasoconstrictive cardiovascular and cerebrovascular events.

What this interaction means in plain words

Eletriptan and Lopinavir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Eletriptan belongs to Central Nervous System; Lopinavir belongs to Anti Virals.

Evidence level: High

Why it happens

CYP3A4 inhibition (mainly by the ritonavir component) reducing eletriptan clearance.

What to do

Avoid eletriptan during and for about 72 hours after lopinavir/ritonavir. Use an alternative triptan such as rizatriptan with attention to cardiovascular risk.
