For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Voriconazole increases eliglustat concentrations, which can prolong PR, QRS and QT intervals and cause arrhythmias. The risk is greatest in CYP2D6 poor or intermediate metabolizers and when combined with CYP2D6 inhibitors.

What this interaction means in plain words

Eliglustat and Voriconazole are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Eliglustat belongs to Miscellaneous; Voriconazole belongs to Anti Fungals.

Evidence level: High

Why it happens

Voriconazole inhibits CYP3A4, a secondary clearance pathway for eliglustat, raising its exposure.

What to do

Avoid in CYP2D6 intermediate or poor metabolizers and with any CYP2D6 inhibitor; in extensive metabolizers reduce eliglustat to 84 mg once daily and monitor ECG.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
