Summary
Elranatamab can cause cytokine release syndrome, which temporarily suppresses CYP enzyme activity, including CYP1A2, the main pathway for tizanidine metabolism. Tizanidine exposure may rise, with more hypotension, bradycardia and sedation, mainly around step-up dosing and during CRS.
What this interaction means in plain words
Elranatamab and Tizanidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Elranatamab belongs to Oncology; Tizanidine belongs to Musculoskeletal.
Why it happens
Cytokine-mediated suppression of CYP1A2 reduces tizanidine clearance.
What to do
Monitor blood pressure, heart rate and sedation during step-up dosing and any CRS episode. Reduce or hold tizanidine if hypotension or bradycardia occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
