Summary
Icatibant blocks bradykinin B2 receptors, and part of the blood pressure lowering of ACE inhibitors depends on raised bradykinin. Icatibant may therefore weaken enalapril's antihypertensive effect. This is mainly theoretical, and icatibant is used short term for ACE-inhibitor or hereditary angioedema attacks.
What this interaction means in plain words
Enalapril and Icatibant have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Enalapril belongs to Cardiovascular; Icatibant belongs to Miscellaneous.
Why it happens
Pharmacodynamic antagonism through bradykinin B2 receptor blockade, opposing the bradykinin-mediated component of ACE inhibitor effects.
What to do
Monitor blood pressure after icatibant use; no dose adjustment is usually needed. If treating ACE-inhibitor angioedema, stop enalapril.
