Summary

Icatibant blocks the bradykinin B2 receptor, and part of the effect of ACE inhibitors is mediated by bradykinin. Co-use may attenuate the antihypertensive effect of perindopril, though the clinical effect is probably small. Icatibant is used short-term for acute angioedema attacks, including ACE-inhibitor induced ones.

What this interaction means in plain words

Amlodipine-perindopril and Icatibant have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.

Amlodipine-perindopril belongs to Cardiovascular; Icatibant belongs to Miscellaneous.

Evidence level: Low

Why it happens

Pharmacodynamic antagonism of bradykinin-mediated effects of ACE inhibition.

What to do

Monitor blood pressure around icatibant use. ACE inhibitor-induced angioedema should prompt stopping perindopril.
