Summary
Etravirine is metabolized by CYP3A4 and CYP2C19, and inducing anticonvulsants can substantially lower its levels, risking virologic failure. Eslicarbazepine's CYP2C19 inhibition could partly raise levels, but the net effect is likely a reduction.
What this interaction means in plain words
Eslicarbazepine and Etravirine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Eslicarbazepine belongs to Central Nervous System; Etravirine belongs to Anti Virals.
Why it happens
CYP3A4 induction by eslicarbazepine increases etravirine clearance, with a possible opposing effect from CYP2C19 inhibition.
What to do
Avoid the combination where possible and choose an antiseizure drug without enzyme-inducing effects. If used together, monitor HIV viral load closely.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
