Summary
Beta-blockers antagonize the bronchodilator effect of indacaterol and may provoke bronchospasm in patients with airway disease. Esmolol is cardioselective and very short-acting, so the effect is usually brief and limited to the infusion period. Reduced response to the bronchodilator may still occur at higher doses.
What this interaction means in plain words
Esmolol and Indacaterol-glycopyrronium can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Esmolol belongs to Cardiovascular; Indacaterol-glycopyrronium belongs to Respiratory.
Why it happens
Pharmacodynamic antagonism at beta2 receptors; esmolol selectivity for beta1 diminishes at higher doses.
What to do
Use esmolol only when clearly needed and at the lowest effective dose, monitoring respiratory status and wheeze. Have a rescue bronchodilator available.
