Proton pump inhibitors like esomeprazole can reduce oral levothyroxine absorption, potentially increasing TSH and reducing clinical efficacy.
Mechanism of action
Pharmacokinetic: Esomeprazole increases gastric pH, impairing dissolution and intestinal absorption of levothyroxine tablets.
Clinical management
Monitor TSH (and free T4 if indicated) about 6–8 weeks after starting, stopping, or changing the dose of esomeprazole; adjust levothyroxine dose as needed. Advise consistent levothyroxine administration on an empty stomach. If control remains unstable, consider switching to a liquid/soft-gel levothyroxine formulation.
