Summary
Oral neomycin alters gut flora that take part in enterohepatic recirculation of estrogens, which could theoretically reduce estradiol exposure. Evidence of reduced efficacy is weak, and contraceptive failure has been reported mainly as isolated cases with other antibiotics.
What this interaction means in plain words
Estradiol and Neomycin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Estradiol belongs to Endocrine; Neomycin belongs to Anti Bacterial.
Why it happens
Suppression of gut bacteria may reduce deconjugation and enterohepatic recycling of estrogens.
What to do
No routine change is needed. Advise patients using estradiol-based contraception of the small theoretical risk, and consider backup contraception during prolonged neomycin courses or if breakthrough bleeding occurs.
