Summary

Estrogens can increase glucuronidation of valproate, which may lower its serum concentrations. In patients whose epilepsy is well controlled, this could allow breakthrough seizures. The effect is variable and is best documented with ethinylestradiol-containing contraceptives.

What this interaction means in plain words

Estradiol and Valproic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Estradiol belongs to Endocrine; Valproic acid belongs to Central Nervous System.

Evidence level: Low

Why it happens

Estrogens may induce UDP-glucuronosyltransferase activity, increasing valproate glucuronide formation and clearance.

What to do

Monitor seizure frequency and valproate concentrations when estrogen therapy is started, stopped or changed. Adjust the valproate dose if control worsens.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
