Summary
Vancomycin is hypothetically able to disrupt gut bacteria that deconjugate estrogens for enterohepatic recycling, which could slightly lower estradiol levels. Convincing evidence of reduced hormonal efficacy or contraceptive failure with vancomycin is lacking.
What this interaction means in plain words
Estradiol and Vancomycin have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Estradiol belongs to Endocrine; Vancomycin belongs to Anti Bacterial.
Why it happens
Possible reduced enterohepatic recirculation of estrogen due to altered gut flora; not firmly established.
What to do
No routine action is needed. Advise patients to report breakthrough bleeding or return of menopausal symptoms.
