Summary
Combining a loop diuretic with a thiazide produces sequential nephron blockade and a strong diuretic response. This can cause marked hypokalemia, hyponatremia, hypomagnesemia, volume depletion and hypotension, especially with an ACE inhibitor on board. Ethacrynic acid also carries ototoxicity risk.
What this interaction means in plain words
Ethacrynic acid and Hydrochlorothiazide-lisinopril can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ethacrynic acid belongs to Cardiovascular; Hydrochlorothiazide-lisinopril belongs to Cardiovascular.
Why it happens
Additive inhibition of sodium reabsorption at the loop of Henle and distal tubule, increasing urinary loss of electrolytes and water.
What to do
Monitor electrolytes, renal function, volume status and blood pressure closely, starting with low doses. Replace potassium and magnesium as needed.
