Summary

Two loop diuretics given together have additive effects on fluid and electrolyte loss, with a high risk of hypokalemia, hypomagnesemia, hypovolemia and ototoxicity. There is rarely a reason to combine them.

What this interaction means in plain words

Ethacrynic acid and Torsemide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Ethacrynic acid belongs to Cardiovascular; Torsemide belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Both inhibit the Na-K-2Cl cotransporter in the thick ascending limb, producing additive diuresis and ototoxic potential.

What to do

Avoid concurrent use; use a single loop diuretic at an adjusted dose, or add a thiazide if more diuresis is needed. If combined, monitor electrolytes, renal function and hearing.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
