For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Ritonavir strongly inhibits CYP3A4 and P-glycoprotein and can markedly raise everolimus exposure. This increases the risk of mTOR inhibitor toxicity such as infections, myelosuppression, hyperglycemia, hyperlipidemia, stomatitis and pneumonitis.

What this interaction means in plain words

Everolimus and Ritonavir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Everolimus belongs to Oncology; Ritonavir belongs to Anti Virals.

Evidence level: High

Why it happens

Ritonavir inhibits intestinal and hepatic CYP3A4 and P-glycoprotein, reducing everolimus clearance and increasing absorption.

What to do

Avoid concomitant use and select an antiretroviral regimen without strong CYP3A4 inhibition. If unavoidable, use only with specialist oversight, major everolimus dose reduction and therapeutic drug monitoring.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
