Summary

Fedratinib is cleared by CYP3A4 and CYP2C19, and fluoxetine inhibits CYP2C19 and, to a lesser extent, CYP3A4. This may raise fedratinib exposure and increase the risk of toxicity such as gastrointestinal effects, anemia and encephalopathy. Fluoxetine is also a moderate CYP2D6 inhibitor-relevant substrate interaction since fedratinib inhibits CYP2D6.

What this interaction means in plain words

Fedratinib and Fluoxetine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fedratinib belongs to Oncology; Fluoxetine belongs to Central Nervous System.

Evidence level: Low

Why it happens

Inhibition of CYP2C19 and CYP3A4 by fluoxetine may reduce fedratinib metabolism.

What to do

Monitor for fedratinib toxicity, including blood counts and neurological symptoms. Consider reducing the fedratinib dose or choosing a different antidepressant.
