Summary
Fedratinib is cleared mainly by CYP3A4, and nicardipine is a CYP3A4 and CYP2C19 inhibitor in vitro. Higher fedratinib exposure could increase GI toxicity, myelosuppression and the risk of encephalopathy, but the combination has not been formally studied.
What this interaction means in plain words
Fedratinib and Nicardipine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fedratinib belongs to Oncology; Nicardipine belongs to Cardiovascular.
Why it happens
Nicardipine may inhibit CYP3A4 and CYP2C19, the enzymes that metabolize fedratinib; the clinical magnitude is not established.
What to do
Monitor for fedratinib adverse effects such as nausea, vomiting, anemia and neurological symptoms, and consider an alternative antihypertensive such as amlodipine. Reduce the fedratinib dose per labeling if toxicity occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
