For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Tranylcypromine can inhibit CYP2C19 and, to a lesser extent, other CYP enzymes, so it may raise fedratinib exposure, which is cleared by CYP3A4 and CYP2C19. Higher fedratinib levels may increase toxicity such as GI effects, myelosuppression and the risk of encephalopathy including Wernicke's.

What this interaction means in plain words

Fedratinib and Tranylcypromine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fedratinib belongs to Oncology; Tranylcypromine belongs to Central Nervous System.

Evidence level: Low

Why it happens

Inhibition of CYP2C19 (and possibly CYP3A4) by tranylcypromine may reduce fedratinib metabolism.

What to do

Consider an alternative to tranylcypromine where possible. If coadministered, monitor for fedratinib adverse effects and thiamine status, and reduce the fedratinib dose per labeling if needed.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
