Summary
Combining a fibrate with a statin raises the chance of myopathy and rhabdomyolysis compared with either drug alone. Fenofibrate carries a lower risk than gemfibrozil because it does not markedly raise statin concentrations, but additive muscle toxicity is still possible, especially with renal impairment, older age or hypothyroidism.
What this interaction means in plain words
Fenofibrate and Pitavastatin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fenofibrate belongs to Cardiovascular; Pitavastatin belongs to Cardiovascular.
Why it happens
Additive myotoxic effects of both drug classes, with little pharmacokinetic interaction for fenofibrate and pitavastatin.
What to do
If the combination is needed for mixed dyslipidemia, use the lowest effective doses and counsel the patient to report unexplained muscle pain or weakness. Check CK if symptoms develop and stop therapy if myopathy is suspected.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
