Summary
Losartan is a weak CYP3A4 inhibitor in vitro, and finerenone is mainly cleared by CYP3A4, so exposure to finerenone may rise slightly. In addition, both losartan and finerenone raise serum potassium, so the combination increases the risk of hyperkalemia, although the thiazide component partly offsets this.
What this interaction means in plain words
Finerenone and Hydrochlorothiazide-losartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Finerenone belongs to Cardiovascular; Hydrochlorothiazide-losartan belongs to Cardiovascular.
Why it happens
Additive potassium-retaining effects from angiotensin receptor blockade and mineralocorticoid receptor antagonism, plus possible minor CYP3A4 inhibition raising finerenone levels.
What to do
Check serum potassium and renal function at the start of therapy and after dose changes of either drug. Hold or reduce finerenone if potassium rises above the labeled thresholds.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
