Summary

Ritonavir strongly inhibits CYP3A4 and markedly raises finerenone exposure. This increases the risk of hyperkalemia, hypotension and renal function decline.

What this interaction means in plain words

Finerenone and Ritonavir are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Finerenone belongs to Cardiovascular; Ritonavir belongs to Anti Virals.

Evidence level: High

Why it happens

Strong CYP3A4 inhibition by ritonavir reduces the oxidative metabolism of finerenone.

What to do

Do not use finerenone with ritonavir or other strong CYP3A4 inhibitors. Choose an alternative antiretroviral or withhold finerenone while the inhibitor is needed.

What to ask your pharmacist

  • Is there an alternative to one of these two medicines for me?
  • If both are essential, who is monitoring me and how often?
  • Which symptoms mean I should stop and seek help straight away?
