Summary

Fluvoxamine is a strong CYP2C19 inhibitor and also inhibits CYP3A4 to a lesser degree, and may considerably raise flibanserin exposure. This increases the risk of hypotension, syncope and sedation.

What this interaction means in plain words

Flibanserin and Fluvoxamine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Flibanserin belongs to Genitourinary; Fluvoxamine belongs to Central Nervous System.

Evidence level: High

Why it happens

Inhibition of CYP2C19 (and CYP3A4) decreases flibanserin metabolism and raises plasma concentrations.

What to do

Avoid combining or use with great caution; flibanserin labeling advises against use with strong CYP2C19 inhibitors. Monitor for hypotension, syncope and sedation if unavoidable.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
