For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Lomitapide is a CYP3A4 substrate, and fluconazole, a moderate CYP3A4 inhibitor, can raise its levels several-fold. This increases the risk of hepatotoxicity and gastrointestinal adverse effects. Labeling contraindicates moderate and strong CYP3A4 inhibitors with lomitapide.

What this interaction means in plain words

Fluconazole and Lomitapide are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Fluconazole belongs to Anti Fungals; Lomitapide belongs to Cardiovascular.

Evidence level: High

Why it happens

Fluconazole inhibits CYP3A4, reducing the metabolism of lomitapide.

What to do

Do not combine them. If antifungal therapy is needed, use a non-CYP3A4-inhibiting alternative or hold lomitapide during a short course.
