Summary
Fluconazole is a moderate CYP3A4 inhibitor (and also inhibits CYP2C9 and CYP2C19) and may increase tamsulosin exposure. Higher levels can cause hypotension, dizziness and syncope, and the risk is greater when combined with a CYP2D6 inhibitor.
What this interaction means in plain words
Fluconazole and Tamsulosin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fluconazole belongs to Anti Fungals; Tamsulosin belongs to Genitourinary.
Why it happens
Fluconazole inhibits CYP3A4, reducing tamsulosin clearance.
What to do
Monitor for orthostatic hypotension during fluconazole therapy and counsel patients to rise slowly. Consider temporary tamsulosin dose reduction if symptoms occur.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
