Summary

Both drugs can prolong the QT interval, so combined use may raise the risk of torsades de pointes, especially with other risk factors. Fluoxetine also inhibits CYP2D6 and may raise beta-blocker exposure and bradycardia, although sotalol is mainly renally cleared.

What this interaction means in plain words

Fluoxetine and Sotalol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Fluoxetine belongs to Central Nervous System; Sotalol belongs to Cardiovascular.

Evidence level: Low

Why it happens

Additive QT prolongation through cardiac potassium channel effects, with possible additional bradycardia.

What to do

Obtain a baseline ECG and monitor QTc after starting or changing the dose, and correct potassium and magnesium. Consider an alternative antidepressant with less QT effect, such as sertraline, if QTc rises significantly.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
