Summary

Levoketoconazole is a strong CYP3A4 inhibitor and can increase systemic exposure to fluticasone furoate and vilanterol. Increased corticosteroid and beta-agonist effects, including adrenal suppression, tachycardia and hypokalemia, may result. Levoketoconazole itself prolongs QT, which adds to arrhythmia concern.

What this interaction means in plain words

Fluticasone-vilanterol and Levoketoconazole can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Fluticasone-vilanterol belongs to Respiratory; Levoketoconazole belongs to Endocrine.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4-mediated metabolism, plus additive QT effects from levoketoconazole and the beta-agonist.

What to do

Avoid if possible or monitor ECG, potassium and heart rate closely. Consider an alternative inhaled regimen less dependent on CYP3A4.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
