Summary
Sotalol is a non-selective beta-blocker that directly opposes the bronchodilating effect of vilanterol and may trigger bronchospasm in patients with asthma or COPD. Sotalol also prolongs the QT interval, and beta-2 agonists can add to this through tachycardia and hypokalemia.
What this interaction means in plain words
Fluticasone-vilanterol and Sotalol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Fluticasone-vilanterol belongs to Respiratory; Sotalol belongs to Cardiovascular.
Why it happens
Pharmacodynamic antagonism at beta-2 receptors, together with additive QT-prolonging and proarrhythmic effects.
What to do
Avoid sotalol in patients with obstructive airway disease who need a beta-agonist; consider another antiarrhythmic. If it is unavoidable, monitor respiratory status, ECG and potassium and magnesium.
What to ask your pharmacist

- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
