Summary
Serotonin syndrome has been reported when 5-HT3 antagonists are combined with SSRIs. Fluvoxamine inhibits CYP1A2 and CYP3A4, which participate in granisetron metabolism, so granisetron exposure could rise. The clinical significance is usually small.
What this interaction means in plain words
Fluvoxamine and Granisetron can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Fluvoxamine belongs to Central Nervous System; Granisetron belongs to Gastrointestinal Tract.
Why it happens
Possible additive serotonergic effect, with CYP inhibition by fluvoxamine possibly increasing granisetron levels.
What to do
Monitor for signs of serotonin toxicity and QT-related symptoms. Stop the agents and treat supportively if serotonin syndrome is suspected.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
