Drug interactions
furosemide + gentamicin
For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.
Overview
Loop diuretics (furosemide) can potentiate the ototoxicity of aminoglycosides (gentamicin) and may increase nephrotoxicity risk, especially with high doses/rapid IV administration, renal impairment, or prolonged therapy.
Mechanism of action
Pharmacodynamic: additive ototoxic effects (furosemide can cause or potentiate cochlear toxicity, particularly with rapid IV dosing), and increased nephrotoxicity risk via diuretic-induced volume depletion concentrating aminoglycoside in renal tubules; no significant pharmacokinetic interaction.
Clinical management
Avoid concomitant use if possible. If necessary: use lowest effective doses, avoid rapid IV furosemide, ensure adequate hydration, perform gentamicin therapeutic drug monitoring (peaks/troughs) and adjust dosing, and monitor renal function (SCr, BUN, urine output) and for auditory/vestibular toxicity; consider baseline and periodic audiometry in high-risk patients.
