Summary
Combining a loop diuretic with a thiazide produces strong sequential nephron blockade, with marked diuresis, volume depletion, hypotension and electrolyte loss. Hypokalemia, hyponatremia and hypomagnesemia can occur, and the ACE inhibitor adds to the risk of renal impairment.
What this interaction means in plain words
Furosemide and Hydrochlorothiazide-lisinopril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Furosemide belongs to Cardiovascular; Hydrochlorothiazide-lisinopril belongs to Cardiovascular.
Why it happens
Additive natriuresis and kaliuresis from blocking sodium reabsorption at different nephron sites.
What to do
Monitor electrolytes, renal function, weight and blood pressure, especially after initiation or dose changes. Reduce diuretic doses if volume depletion develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
