Summary
Gemfibrozil can increase exposure to glyburide and may cause more frequent or prolonged hypoglycemia. Glyburide is metabolized in part by CYP2C9 and CYP3A4, and gemfibrozil may reduce its clearance. Gemfibrozil may also raise exposure through transporter effects.
What this interaction means in plain words
Gemfibrozil and Glyburide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Gemfibrozil belongs to Cardiovascular; Glyburide belongs to Endocrine.
Why it happens
Inhibition of CYP2C9 (and possibly hepatic uptake transporters) reduces glyburide clearance, along with possible protein-binding displacement.
What to do
Monitor blood glucose closely when the combination is started, changed or stopped. Reduce the glyburide dose if hypoglycemia occurs, or consider an alternative lipid-lowering agent such as a statin.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
