Summary
Both thiazide-related electrolyte loss and aminoglycoside nephrotoxicity may impair renal function, and the ACE inhibitor and volume depletion add further risk. Hypokalemia and hypomagnesemia can also occur with gentamicin and may worsen with the diuretic.
What this interaction means in plain words
Gentamicin and Hydrochlorothiazide-lisinopril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Gentamicin belongs to Anti Bacterial; Hydrochlorothiazide-lisinopril belongs to Cardiovascular.
Why it happens
Aminoglycoside tubular toxicity combined with diuretic-induced volume depletion and ACE inhibitor effects on glomerular filtration increases kidney injury risk.
What to do
Monitor serum creatinine, potassium and magnesium and gentamicin levels. Consider holding the diuretic and ACE inhibitor if volume depletion or renal decline occurs.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
