Summary

Azole antifungals inhibit the metabolism of sulfonylureas and can raise gliclazide levels, causing hypoglycemia. Posaconazole is mainly a CYP3A4 inhibitor, so the effect on CYP2C9-metabolized gliclazide is likely smaller than with fluconazole or miconazole.

What this interaction means in plain words

Gliclazide and Posaconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Gliclazide belongs to Endocrine; Posaconazole belongs to Anti Fungals.

Evidence level: Low

Why it happens

Possible inhibition of hepatic metabolism of gliclazide, mainly CYP2C9 with some CYP2C19 involvement.

What to do

Monitor blood glucose during posaconazole therapy and reduce the gliclazide dose if hypoglycemia occurs.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
