Summary

Glofitamab causes cytokine release that can transiently suppress CYP450 enzyme activity. This may raise exposure to CYP substrates such as repaglinide, with a risk of hypoglycemia. The effect is greatest around initial doses and during cytokine release syndrome.

What this interaction means in plain words

Glofitamab and Metformin-repaglinide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Glofitamab belongs to Oncology; Metformin-repaglinide belongs to Endocrine.

Evidence level: Low

Why it happens

Cytokine-mediated suppression of CYP enzyme expression decreases repaglinide metabolism.

What to do

Monitor blood glucose during the first doses of glofitamab and during any cytokine release syndrome. Lower the repaglinide dose if hypoglycemia occurs.
