Summary

Voriconazole can prolong the QT interval and granisetron has a comparable effect, so concurrent use may increase risk of torsades de pointes. Voriconazole also inhibits CYP3A4, which contributes to granisetron metabolism and may modestly raise its exposure.

What this interaction means in plain words

Granisetron and Voriconazole can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Granisetron belongs to Gastrointestinal Tract; Voriconazole belongs to Anti Fungals.

Evidence level: Moderate

Why it happens

Additive QT prolongation, possibly with reduced CYP3A4-mediated clearance of granisetron.

What to do

Obtain a baseline ECG and correct potassium, magnesium and calcium. Monitor the QTc during combined therapy, particularly in patients with other risk factors.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
