Summary

Lomitapide is a sensitive CYP3A4 substrate and haloperidol is a weak CYP3A4 inhibitor, so lomitapide exposure may rise. Higher lomitapide levels increase the risk of hepatotoxicity and gastrointestinal adverse effects. Haloperidol is itself a weaker inhibitor than the strong and moderate inhibitors that are formally contraindicated.

What this interaction means in plain words

Haloperidol and Lomitapide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Haloperidol belongs to Central Nervous System; Lomitapide belongs to Cardiovascular.

Evidence level: Low

Why it happens

Haloperidol weakly inhibits CYP3A4, reducing the first-pass and systemic clearance of lomitapide.

What to do

Monitor liver enzymes and gastrointestinal tolerability, and consider reducing the lomitapide dose per labeling for weak inhibitors. Reassess the lipid-lowering plan if transaminases rise.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
