Summary

Haloperidol prolongs the QT interval and carries a known risk of torsades de pointes. Rilpivirine at higher than standard doses also prolongs QT, so the combination may add to arrhythmia risk, especially with other risk factors.

What this interaction means in plain words

Haloperidol and Rilpivirine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Haloperidol belongs to Central Nervous System; Rilpivirine belongs to Anti Virals.

Evidence level: Moderate

Why it happens

Additive blockade of cardiac potassium channels (hERG) leading to QTc prolongation.

What to do

Obtain a baseline ECG and monitor QTc during co-therapy, and correct hypokalemia and hypomagnesemia. Consider an alternative antipsychotic or antiretroviral if QTc becomes markedly prolonged.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
