Summary

NSAIDs can blunt the antihypertensive effect of ACE inhibitors and thiazides and, together with them, raise the risk of acute kidney injury, especially with volume depletion or in older patients. NSAIDs can also contribute to hyperkalemia with the ACE inhibitor.

What this interaction means in plain words

Hydrochlorothiazide-lisinopril and Lornoxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Hydrochlorothiazide-lisinopril belongs to Cardiovascular; Lornoxicam belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAIDs inhibit renal prostaglandin synthesis, causing sodium and water retention and reduced glomerular perfusion, which counteracts the antihypertensive effect and impairs renal function when combined with ACE inhibition and diuresis.

What to do

Monitor blood pressure, serum creatinine and potassium after starting the NSAID and periodically thereafter. Keep the patient hydrated and use the lowest NSAID dose for the shortest time.

What to ask your pharmacist
