Summary

Torsemide and hydrochlorothiazide both promote urinary potassium and magnesium loss, and sequential nephron blockade can produce a strong diuresis. This can cause hypokalemia, hyponatremia, volume depletion and hypotension, with a risk of arrhythmias. Lisinopril may lessen potassium loss but increases the risk of first-dose hypotension and renal impairment when volume depleted.

What this interaction means in plain words

Hydrochlorothiazide-lisinopril and Torsemide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Hydrochlorothiazide-lisinopril belongs to Cardiovascular; Torsemide belongs to Cardiovascular.

Evidence level: High

Why it happens

Additive natriuresis and kaliuresis from loop and thiazide diuretic action at sequential nephron sites.

What to do

Monitor electrolytes, renal function, weight and blood pressure, and consider lowering diuretic doses if volume depletion appears. Replace potassium and magnesium as needed.

What to ask your pharmacist
