Summary
Both are class III antiarrhythmics that markedly prolong repolarization, and together they substantially raise the risk of torsades de pointes. Ibutilide labeling advises against giving it with other drugs that prolong QT, and class III agents are typically withheld before ibutilide use.
What this interaction means in plain words
Ibutilide and Sotalol are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Ibutilide belongs to Cardiovascular; Sotalol belongs to Cardiovascular.
Why it happens
Additive IKr potassium channel blockade causing marked QT prolongation.
What to do
Do not combine; hold sotalol before ibutilide cardioversion for an interval based on its half-life and renal function, as advised in labeling. Use an alternative cardioversion strategy if sotalol cannot be withheld.
