Summary
Tucatinib is a strong CYP3A inhibitor, and ifosfamide is a prodrug that depends on CYP3A4 (and CYP2B6) for both activation and for formation of neurotoxic and nephrotoxic chloroacetaldehyde. Inhibition may shift ifosfamide metabolism and change the balance between active and toxic metabolites, with uncertain clinical effect.
What this interaction means in plain words
Ifosfamide and Tucatinib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Ifosfamide belongs to Oncology; Tucatinib belongs to Oncology.
Why it happens
Tucatinib inhibits CYP3A4, altering the metabolic activation and toxic-metabolite formation of ifosfamide; the net effect is not well characterized.
What to do
Monitor for neurotoxicity, nephrotoxicity and reduced efficacy, and consider an alternative to tucatinib if feasible. Reassess renal function and mental status during each ifosfamide cycle.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
