Summary

Tucatinib inhibits CYP3A4 and can raise cisapride concentrations, increasing the risk of QT prolongation and life-threatening arrhythmias such as torsades de pointes. Cisapride is contraindicated with strong or moderate CYP3A4 inhibitors.

What this interaction means in plain words

Cisapride and Tucatinib are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Cisapride belongs to Gastrointestinal Tract; Tucatinib belongs to Oncology.

Evidence level: Moderate

Why it happens

Inhibition of CYP3A4-mediated cisapride metabolism raises plasma levels and QT risk.

What to do

Do not coadminister; use an alternative prokinetic that is not a CYP3A4 substrate with QT risk.

What to ask your pharmacist
