Summary
Sotalol is a non-selective beta-blocker that antagonizes the bronchodilating effect of salbutamol and may provoke bronchospasm in asthma or COPD. Both drugs also affect QT and potassium, raising the risk of torsades de pointes when salbutamol causes hypokalemia.
What this interaction means in plain words
Ipratropium-salbutamol and Sotalol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Ipratropium-salbutamol belongs to Respiratory; Sotalol belongs to Cardiovascular.
Why it happens
Pharmacodynamic antagonism at beta2-receptors plus additive QT prolongation and beta-agonist-induced hypokalemia.
What to do
Avoid sotalol in patients with significant reactive airway disease needing salbutamol; consider an alternative antiarrhythmic. If used together, monitor ECG, potassium and magnesium and assess bronchodilator response.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
